Medical: On 3/3 exam Doctor noted: "Senior exam. Teeth have heavy calculus and gingival recession associated with 108, 109, moderate diffuse calculus. There is a 2-3 cm fleshy black pigmented cutaneous growth on crown of head. Good body condition. Scared but sweet guy. Assessment: Periodontal disease, esp associated with 108, 109 ~2-3 cm cutaneous growth, crown of head" On 3/1 intake staff noted: "Approximately 8-year-old unaltered male. Teeth are stained. Bright, alert, responsive, reactive to touching back legs and restraint."
