Even if you just moved, you need to have a vet you went too. That means, if you have a ten year old cat, he should have ten vet visits on record. If you have HAD pets in the past, please list the vet office you went too. If you have never had pets, you MUST list three non family references. 2) Please fill out all the questions and give accurate and correct information. 3) Applicants must be at least 21 years old. 4) You must live within an hour and a half of Pittsburgh.
